Add PmWiki writer
- Alter MediaWiki writer to be subclassable to suit the PmWiki changes.
- Make a trivial subclass of MediaWiki to produce PmWiki.
- PmWiki (like MediaWiki) still needs to be enabled by using the
writer_map config variable.
- I should look into unifying all of the differennt writers even more.
